Effective July 2, 2001, Public Law 106-113, Section 236 requires that U.S. passport applications for children under the age of 16 require the consent of both parents or legal guardians. The purpose of this requirement is to reduce the possibility that a U.S. passport will be used to facilitate an international parental child abduction.
4. Proof of the child's relationship to the parents or guardians (certified U.S. Birth Certificate; or certified foreign Birth Certificate with translation, if necessary; or Report of Birth Abroad; or Adoption Decree; or certified or original guardianship documents).
5. If one parent is absent and unable to sign DSP-11, a notarized Form DS-3053 is required. (Available at the Consular Section or online at: http://travel.state.gov/passport/forms/ds3053/ds3053_846.html) The absent parent must submit a copy of his or her passport.
6. The child's most recent passport.
7. One current passport photo
8. Fee of $60 payable in Estonian kroons (cash), U.S. cash or credit card.
